We are excited to introduce our “Healthy Kids Clinic” in your child’s school this school year! To ensure complete transparency and to assist parents in making the best health decisions possible for their students, we have prepared a list of frequently asked questions to address any lingering questions related to this exciting program:

Who is Cumberland Family Medical Center, Inc.?
Cumberland Family Medical Center, Inc. – our Healthy Kids Clinic parent organization – is a Federally Qualified Health Center with community health center locations throughout south-central and a portion of north-central Kentucky. In 2017, CFMC, Inc. was recognized by the U.S. Department of Health and Human Services as a National Quality Leader among the top 1% of community health centers in the U.S. Should you permit for your child to be seen at school in the Healthy Kids Clinic, you are welcome to be there for the visit if you choose.

What services are available through Healthy Kids Clinic?
Healthy Kids Clinic offers traditional school nursing services with the addition of an on-site medical provider. With your permission, the medical provider can see your child for sick visits, well-child exams, sports physicals, immunizations, laboratory services, and injections. This is exactly like seeing a medical provider outside of the school setting, without the inconvenience of students being absent or parents missing work.

Will you see my child for an office visit without me knowing?
No. Even if you sign the permission form stating that you will allow your child to be seen by the Nurse Practitioner, we will always call you BEFORE the child is seen to double-check and make sure you are aware and give permission. Parents/guardians are always welcome to attend the visit if you so desire.

Will I still be able to take my child to his/her primary care physician?
Absolutely. We encourage you to have a family medical home and maintain regular checkups for both yourself and your children. Our Healthy Kids Clinic is not intended to replace your family physician or child’s pediatrician; it is merely an option for speedy treatment when needed and desired to keep your child healthy, in school, and reduce the amount of time parents may have to spend away from work.

What about my co-pay?
In the event that your child is seen by the nurse practitioner with your permission in the Healthy Kids Clinic, if applicable, your insurance co-pay will be applied to that visit just the same as if you were at your regular doctor’s office. You DO NOT have to make this payment at school. You will receive an invoice from Cumberland Family Medical Center, Inc. in the mail for that co-pay amount.

Is this program for all types of insurance?
The Healthy Kids Clinic is for all kids! Every student in the school system is equally important to us! All students can be seen in the Healthy Kids Clinic with parental permission regardless of their family income or insurance status. If your child does not have insurance please contact the Healthy Kids Clinic and they will explain our sliding fee scale program.

Will my child receive medicines without my permission?
No child will receive any type of treatment without parental permission on the day of service. Any prescription that the Nurse Practitioner may prescribe for your child will be faxed to the pharmacy of your choice to be filled at the parent’s discretion. The Healthy Kids Clinic does not dispense any prescription medications or birth control products.

Will school officials have access to my child’s personal or health information?
The same rules that govern patient privacy in a regular physician’s office are the same rules that apply in the Healthy Kids Clinic. No unauthorized school personnel will have access to any personal health records without the express written consent of the parent or guardian.

Will this program go away?
Cumberland Family Medical Center, Inc. is here to stay! We anticipate a long and healthy partnership with our local school systems that will result in healthier kids and a healthier community.

Can my child continue to receive school nurse services without signing a consent for them to see the Nurse Practitioner?
Absolutely! The Healthy Kids Clinic is just one option available to help keep your child healthy and safe while at school. Traditional school nurse services like getting an ice pack, receiving daily medication, a band-aide, etc. will be continued the same as always. We would like for you to please complete and return the registration form and medical history forms, however, as these have the necessary information regarding allergies, etc. and also allow for you to give permission for your child to receive an over-the-counter medication such as Tylenol or a cough drop, etc. if needed. You as the guardian have the option to choose what services you want.

If you did not complete the Healthy Kids Clinic permission forms, please do so as soon as possible and return them to your child’s school. For more information or to schedule an appointment for your child, contact your school’s Healthy Kids Clinic toll-free at (844) 435-0900.

Thank you! We look forward to a healthy and productive school year and are confident that the Healthy Kids Clinic will play a large role in this success!
